Rachel Dent
Biographical Info
I am a Registered Nurse with over 30 years of clinical experience, including more than 12 years in senior leadership within Ontario’s long-term care sector. My expertise centers on the investigation and evaluation of complex clinical cases, with a focus on standards of care in long-term care environments.
As a Director of Care/Associate Director of Care, I lead investigations into critical incidents, resident care concerns, and workplace injuries. I conduct detailed reviews of medical records, video footage, and clinical documentation, and perform witness interviews to assess causation, identify deviations from accepted standards, and support evidence-based, defensible conclusions. My role requires advanced analytical skills and in-depth knowledge of regulatory frameworks, including Ministry of Long-Term Care and Ministry of Labour requirements, College of Nurses of Ontario standards, PHIPA, and CIHI, along with extensive experience investigating and managing WSIB cases.
I have advanced competencies in medical documentation review and the management of complex cases involving frailty, chronic illness, wounds, and cognitive impairment. I am skilled in synthesizing complex clinical information into clear, concise reports.
Currently pursuing certification through the Legal Nurse Consultant Academy of Canada, I am focused on long-term care litigation and prepared to provide objective, evidence-based analysis to support legal professionals in evaluating standards of care and case merit.
